تنشر سفارة إيطاليا بالجزائر استمارتها الخاصة مجانًا (Dichiarazione garanzia e/o alloggio): استعملها إذا طلبتها القنصلية أو مركز التأشيرات. يتبع هذا الخطاب بنودها لكنه لا يحل محلها. أرفق نسخة من بطاقة تعريفك أو رخصة إقامتك. وإذا كنت الضامن المالي، فتلزم الإثباتات المذكورة في تلك الاستمارة. وبعد الوصول، يجب التصريح بالاستضافة لدى السلطات في أجل 48 ساعة.

Il/La sottoscritto/a si impegna a farsi carico, per tutta la durata del soggiorno, di [Ce que vous prenez en charge (hébergement, repas...)].
Il/La sottoscritto/a dichiara di essere a conoscenza dell'obbligo di comunicare l'ospitalità all'autorità di pubblica sicurezza entro 48 ore, ai sensi dell'art. 7 del D.Lgs. 286/1998. La presente dichiarazione è rilasciata a sostegno della domanda di visto e si allega copia del documento d'identità del/della sottoscritto/a.
